How to Register with the Surgery

To register with the practice you need to fill in a GMS1 form and a Practice Registration form which can be downloaded below or picked up from reception area. Please then drop them in at the surgery. You will need to bring with you your current passport, a utility bill or other proof of address within our area, and your National Insurance card/NHS number.

You may like to book an appointment with one of the healthcare assistants for a new patient check, which involves checking your medical history and medication.
