Repeat Prescriptions

Repeat prescriptions may be obtained by sending or faxing:

1. The request slip attached to prescriptions which are regularly requested.
2. Full details in writing of the medication required, together with patient’s name, address and date of birth.
3. Prescription request forms which are available at the surgery.

PLEASE NOTE THAT REQUESTS BY TELEPHONE CANNOT BE ACCEPTED

All requests for authorised medication will be ready in 2 full working days.

The prescription may be collected by the patient or his/her representative, or by certain pharmacists by prior arrangement. If a stamped addressed envelope is provided, we will post the prescription to you.
