Pre-travel questionnaire
The link below will take you to a downloadable questionnaire which you must submit at least 8 weeks* before your departure date and a separate questionnaire must be completed for each person travelling (including children).
*If you are travelling at short notice you should speak to the Reception Staff who will advise if you can still be seen or need to make an appointment with a Private Travel Clinic.
This can then be emailed to
doctortoovey@nhs.net or you can print it off and post/bring it to the surgery. Please note that, if emailing it will be sent via normal email and so won’t be sent securely.
